EO-364 – “OJT Enrollment Form” Access

PennDOT has advised that the EO-364 – “OJT Enrollment Form” has been revised and can be accessed from the PennDOT and/or BEO Homepage.

Please contractors with projects containing: “SPECIAL PROVISIONS ITEM 1999-9999 – TRAINEES” should utilize the revised EO-364 form as of today. Note: the (02-07) version of the EO-364 is no longer available on-line.
